Center for Comprehensive Services (CCS) - Florida provides individualized outcome driven post-acute rehabilitation services for people with brain and spinal cord injuries as well as other neurological conditions. We provide a continuum of services including active rehabilitation (CCS - Tampa & CCS - Jacksonville/St. Augustine) and Supported Living (CCS - BayView).  (For more information download our Informational PDF)

LOCATION AND SETTING:

CCS - FL has 4 Centers serving the needs of people with ABI and other neurological disabilities. 

  • CCS - Tampa located in Lutz, Florida in a culturally-diverse community that provides easy access to community colleges, and the newly constructed YMCA.  Our transdisciplinary treatment team includes rehabilitation specialists who are fluent in English and Spanish.
  • CCS Orlando at Avalon Park located in the Keith A. Ewing Medical Building of Avalon Park – a 1,860-acre neighborhood in southeast Orange County.
  • CCS - Jacksonville located in St. Augustine, Florida is serving people in northeast Florida and the surrounding region. 
  • Bayview in Sarasota provides Supported Living programming in a beautiful setting near Sarasota Bay.

PROGRAM DETAILS: 

NeuroRehabilitation Program: 

The NeuroRehabilitation program is designed to maximize neurological recovery. Individualized functional goals are designed to help persons with acquired brain injury return home, to their communities, and to work or school. Individual programs offer specific goals relating to medical, physical, cognitive, and emotional areas of concern. A comprehensive evaluation is facilitated across therapeutic disciplines prior to the establishment of functional outcome goals. Our functional therapy approach combines opportunities for therapeutic follow-through, skill acquisition and practice in real-life settings with family training and comprehensive discharge planning.

Services may be provided in intensive residential, small group congregate, or apartment settings with varying degrees of support, as well as in outpatient or day treatment settings.

NeuroBehavioral Program:

 NeuroBehavioral programming provides rehabilitation for persons whose behavioral issues stand in the way of them accessing more traditional rehabilitation options or integrating successfully into the life of their community. NeuroBehavioral programming is delivered in our Cypress residence, which was specially built in 2002 to offer persons with intense behavioral needs the opportunity to receive treatment in a community based setting.  Our NeuroBehavioral treatment teams use proactive teaching approaches based on positive reinforcement or opportunities that support desired behavior.

Our interventions are based on positive reinforcement or appropriate levels of staff support that encourages desired behavior.  Participants served in our Neurobehavioral model also are able to receive other clinical services (i.e. PT, OT, Speech etc) if warrented and appropriate.  Each participant is evaluated by our Behavior Analyst and Neuropsychiatrist.  An individualized Behavior Plan is developed taking into account the goals of the participant.  Interventions are continuously evaluated and modified as individuals progress.

Respite:

A brain injury forever changes the life of a person with the injury and the lives of family members, loved ones, and friends. When a person living a full and independent life suddenly becomes dependent on family members or others for their daily care, significant stresses are placed both on the family system and the person living with brain injury. Customized respite care can be arranged as needed.

Outpatient and Day Treatment :

CCS-Tampa provides outpatient and day-treatment services for adults with neurologic diagnoses including: traumatic brain injury, stroke, tumor, aneurysm, anoxia, spinal cord injury and disease process. The program is designed to support individuals in their medical and physical recovery which began in the hospital, while assisting them in facing daily life issues. Each person's program addresses successful completion of home and community tasks.  Services available include, Occupational Therapy, Physical Therapy, Speech and language Pathology, Behavior Analysis, Neuropsychological Testing, Case Management and Nursing.

Supported Living:

The Supported Living Program is designed to help people live as full and productive a life as possible. Individualized programs and services are provided in home-like community settings. Programs are designed to enable each participant to preserve the goals reached in earlier stages of rehabilitation and to sustain their preferred quality of life. Opportunities for continued skill building are fostered through involvement in the life of the community and the integration of social productive and recreational endeavors into each participant’s daily life. Our flexible, interconnected system of therapeutic options and community resources are as diverse as the persons we serve.

CCS - BAYVIEW  provides Supported Living

As part of the CCS - Tampa continuum of services, BayView’s unique NeuroPsychoSocial Model is a holistic approach designed to address the emotional, medical and social needs of persons with neurocognitive and neurobehavioral impairments. BayView’s primary focus is to promote independence through guidance, teaching and monitoring in community settings.

LOCATION AND SETTING

Located in the community of Sarasota, Florida, CCS - BayView sits on the shores of the Sarasota Bay.  BayView provides 12 private rooms, cable TV, phone and internet access, a pool and access to the Sarasota Bay.
